Cheshire/ Sullivan County:
Monadnock Developmental Services is looking for an Adult Service Coordinator Case Manager for adults with disabilities. This added position is to join a growing team of approximately 20 to work with adults with disabilities. The position is based out of our Keene office for 37.5 hours a week. The caseload will be based out of Cheshire and parts of Sullivan County.

Successful candidates will have strong advocacy skills, ability to work with a variety of teams, network with the community, facilitate meetings, assist with negotiation of budgets and problem solve. Candidates should be organized, have the ability to multitask, and possess strong conflict management, computer and communication skills.

The position entails monthly contact with individuals, families and providers on caseload. Candidate would coordinate and facilitate team meetings and annual service agreements in accordance to regulatory guidelines. Candidate would be responsible for coordinating, monitoring and reviewing quality of services provided through visits and review of documentation.

A Bachelor’s is preferred and a minimum of 2-4 years of experience in human services. Must have experience with individuals with disabilities. Driving and background check required.
