The Bureau of Developmental Services is conducting a survey for all families and guardians to complete about the service your family member receives from Monadnock Developmental Services and its contracting vendor agencies. The survey will be available until February 29, 2020. Responses will be sent directly to the state, and are confidential. This survey is intended for families and guardians of those receiving services from MDS.

What is Redesignation?
Every five years, each Area Agency must be redesignated by the State of New Hampshire to continue as an Area Agency. The redesignation process is a comprehensive evaluation that reviews many aspects of service delivery quality and compliance. The review is required by state regulation and is intended to determine how well the Area Agencies are meeting their responsibilities.
• An Area Agency is required by State Law RSA 171-A and He-M 505 to be designated by the State of New Hampshire, Department of Health and Human Services.
• The formal process is completed for each Area Agency every five years.

What is the Area Agency Strategic Plan?
MDS is updating its Strategic Plan, and your input in this process is very important. This survey, in addition to the questions being asked for redesignation, also asks your feedback to help guide revision of the Area Agency Plan.
